Understanding the Challenges of Head and Neck Skin Cancer

As a surgeon specialising in the delicate and complex field of head and neck reconstruction, I am acutely aware of the dual challenges that skin cancers present: the need to remove tumours completely and the equally important need to preserve function and appearance to ensure patients can eat, speak, and look as close to normal as possible.

During my time at the world-renowned Peter MacCallum Cancer Centre in Melbourne, Australia, I had the privilege of being involved in the cutting-edge DISCERN (Deep Sequencing in Cutaneous Squamous Cell Carcinomas) Trial, exploring novel approaches to one of the most common – and sometimes most aggressive – forms of skin cancer: cutaneous squamous cell carcinoma (CSCC).

Traditional Treatment Approaches

Traditionally, the mainstay of treatment for CSCC has been surgical excision, often combined with radiotherapy in higher-risk cases. Surgery is highly effective, but for patients with advanced or recurrent disease, the risk of recurrence and spread remains a significant concern. This is where innovative research in immunotherapy is reshaping the landscape.

Exploring Immunotherapy with Surgery: The DISCERN Trial

I had the opportunity to participate in the DISCERN trial, which investigated the combination of immunotherapy together with surgery. Immunotherapy works by harnessing the patient’s own immune system to recognise and attack cancer cells; a strategy that has already transformed treatment for melanoma and other cancers.

A Multidisciplinary Approach

At the Peter MacCallum Cancer Centre, our team combined expertise from surgical oncology, medical oncology, radiation oncology, and reconstructive surgery. Each patient’s treatment was carefully tailored, with meticulous attention to both function and appearance. As a reconstructive surgeon, my role was not only to remove the tumour safely, but also to plan and perform reconstruction that optimises form and function; ensuring patients could speak, eat, and express themselves confidently after surgery.

Results and Patient Outcomes

The findings from the trial are very promising. Patients tolerated the combined approach well, and some experienced remarkable tumour shrinkage before surgery. This allowed for less invasive procedures, quicker recovery, and improved overall outcomes. While long-term results are still being evaluated, this represents a significant step forward in managing high-risk head and neck skin cancers.

  • Shrinking the tumour before surgery: 73% of patients’ cancers shrank significantly after the immunotherapy, meaning the surgery could be smaller and less invasive.
  • Good safety: The treatment didn’t cause major side effects or delay surgery.
  • Better outcomes: In patients whose tumours responded completely to the drug, surgeons were able to remove less tissue, preserving more normal function and appearance.

 

Why This Matters

This approach has the potential to transform the way we treat high-risk skin cancers. By “priming” the immune system before surgery, we may be able to:

  • Preserve function and aesthetics: By reducing tumour size preoperatively, more conservative surgical approaches can be employed, leading to better functional and aesthetic outcomes.
  • Enhance disease control: Giving immunotherapy before surgery may help the body target tiny cancer cells that have spread, lowering the chance of the cancer coming back and improving long-term outcomes.
  • Tailor Surgical Planning: The response to immunotherapy can inform surgical strategies, allowing for more precise and individualised treatment plans.

 

The Importance of Innovation and Collaboration

Research like this highlights the power of collaboration, innovation, and patient-centred care. By combining immunotherapy with surgery, we are beginning to see ways not only to extend survival, but also to improve the quality of life for patients facing these challenging cancers.

The Future 

While the results of the DISCERN trial are promising, further research is necessary to validate these findings and determine the optimal integration of immunotherapy into clinical practice. Ongoing studies will provide valuable insights into the long-term benefits and potential challenges associated with this treatment.

As we continue to explore these novel therapies, my hope is that more patients will benefit from treatment strategies that are as sophisticated as they are compassionate, combining the best of surgical precision with the transformative potential of the immune system.
